- General Unique Pool: can drop from every Lair Boss encounter (no dedicated boss)
- random drop - not listed on any Lair Boss loot table in the community loot-table cheat sheet this dataset uses; no official Blizzard list confirms it
- Can also be crafted into a Unique Charm (Horadric Cube > Craft Unique Charm) per 3.2.1 notes; any Unique Charm has a small chance to drop at Mythic quality.
- Horadric Cube: Item Transmutation (Upgrade to Unique) - Common item of the same slot + 1 Enhanced Primordial Dust + 10 Primordial Dust (random Unique of that slot)
- General Unique Pool - every Lair Boss (Initiate, Greater, Exalted, Belial) can drop it in addition to their dedicated table (one guide's Boss Loot Table Cheat Sheet)
