- Horadric Cube: craft with Resplendent Sparks / Runes; S15: Upgrade Direct Item to Mythic ('Crafted' tag)
- Echo of Mephisto: 3.2.1 'Increased overall loot quality for Echo of Mephisto, including a guaranteed Mythic Drop'.
- All Tormented Lair Bosses (5 chances at 1.5% each per kill, one guide); Greater Lair Bosses 2x rate; Belial highest
- Blacksmith: craft with Resplendent Sparks (general Mythic cache; Iconic Mythics have their own cache at the same cost since 3.2.1)
- Any Lair Boss hoard (every Mythic drops from every Uber/Lair boss at the same rate; no boss-specific Mythic - one guide's Bosses page)
